  • Completion of Death Claim Form
  • Original death certificate of insured person
    If the Life Insured passes away in Mainland China, please provide:
    • Notarized Death Certificate
    • Proof of cancellation of the Hong Kong Identification Card issued by the Hong Kong Immigration Department
  • Certified true copy of Hong Kong Identity card of the Life Insured/Beneficiary/Claimant
    • If the beneficiary/claimant is not a Hong Kong Permanently Identify Card Holder, please provide us with nationality proof (e.g. Certified true copy of passport or other identity document(s)
  • Proof of relationship between Life insured and Claimant/Beneficiary
  • Return of original Life Insurance Policy
    If policy is lost, please declare in the Claim Form.
  • Proof of residential address and permanent address (if different from the residential address ) of the claimant/beneficiary
    According to the Anti-Money Laundering and Counter-Terrorist Financing (Financial Institutions) Ordinance ("the AMLO"): Please provide any Tax Notice/ Towngas/ Electric bill or Bank statement etc., issued in the last 3 months.
  • Death Investigation report or post mortem report, if any
    If the death was due to accident, please provide relevant report.
  • Letter of Administration
    If no beneficiary has been assigned, the beneficiary will be the legal representative.
Remarks:

Original bills and receipts (e.g. medical/ hospital receipts) must be submitted together with any of the above claim forms. Clinical diagnosis must be endorsed on all bills and receipts by your attending doctor. Attending Doctor's referral letter should be submitted if you are claiming for Specialist Consultation, Laboratory Test and Long Term Medicine reimbursement. Your request will be processed immediately upon receipt of the form with complete information. You will receive our written reply within 10 working days.

When submitting a claim through MyCigna HK, please have the below documents ready.

Hospitalization / Medical Expenses Claim

  • Accident or Medical Expenses Doctor Form completed by your attending doctor
  • Medical Receipt(s)
  • Hospital statement of charges / invoice / bill with breakdown of charges
  • Hospital Discharge Summary or any document(s) issued by doctor/hospital with diagnosis proof
  • Identity card copy of the Policyholder
  • If you have submitted a claim to other insurer(s), please provide a copy of the relevant Compensation Breakdown
Guarantee of Payment (Applicable to Choice and Elite)

For application of Guarantee of Payment (GOP) before hospitalization please have your admission note ready but if you don't have one you can still submit a claim.

When submitting a claim through My cigna or the app please have the below documents ready:

  • You will need to have your doctor complete an Accident or Medical Expenses Doctor Form (sample)
  • Statement of charges from the hospital/Hospital Invoice 
  • Hospital Discharge Summary or Any dcouments issued by doctor/hospital with diagnosis
  • Identity card copy of the Policyholder

  • Completed Disability Claim Form from your doctor
  • Related medical documentation issued by attending physician/ hospital
  • Sick Leave certificate from your employer
  • Identity card copy of the Policyholder
  • Proof of residential address and permanent address(if different from residential address) within 3 months of the Policyholder
