  3. Only policyholders of CIGNA HealthFirst Medical Insurance Series are entitled to CIGNA Health and Wellness Program.
  4. CIGNA Health & Wellness Program: Terms & Conditions:
    1. Policyholders should call QHMS’s booking hotline at 28513303 to make appointment before enjoying the service.
    2. Policyholders must present CIGNA HealthFirst Medical Card to enjoy the offer.
    3. Policyholders who have successfully made appointment must show up on time. For rescheduling, policyholders should call QHMS’s hotline 24 hours in advance.
    4. Each policyholder can enjoy one offer during every policy year.
    5. Such offer cannot be exchanged with cash and cannot be used in conjunction with other promotional offers.
    6. In case of disputes, the decision of QHMS shall be final.
